The breach was confirmed by the U.S. Department of Health and Human Services and details reveal that hackers accessed files which may contain full names, dates of birth, Social Security numbers, driver’s licenses, medical record numbers, diagnoses, lab results, and healthcare claims. The healthcare provider disclosed that the unauthorized access occurred on or around July 17, 2025.
